Mentally ill flood ER as states cut services (Reuters)

CHICAGO/NEW YORK (Reuters) ? On a recent shift at a Chicago emergency department, Dr. William Sullivan treated a newly homeless patient who was threatening to kill himself.

"He had been homeless for about two weeks. He hadn't showered or eaten a lot. He asked if we had a meal tray," said Sullivan, a physician at the University of Illinois Medical Center at Chicago and a past president of the Illinois College of Emergency Physicians.

Sullivan said the man kept repeating that he wanted to kill himself. "It seemed almost as if he was interested in being admitted."

Across the country, doctors like Sullivan are facing a spike in psychiatric emergencies - attempted suicide, severe depression, psychosis - as states slash mental health services and the country's worst economic crisis since the Great Depression takes its toll.

This trend is taxing emergency rooms already overburdened by uninsured patients who wait until ailments become acute before seeking treatment.

"These are people without a previous psychiatric history who are coming in and telling us they've lost their jobs, they've lost sometimes their homes, they can't provide for their families, and they are becoming severely depressed," said Dr. Felicia Smith, director of the acute psychiatric service at Massachusetts General Hospital in Boston.

Visits to the hospital's psychiatric emergency department have climbed 20 percent in the past three years.

"We've seen actually more very serious suicide attempts in that population than we had in the past as well," she said.

Compounding the problem are patients with chronic mental illness who have been hurt by a squeeze on mental health services and find themselves with nowhere to go.

On top of that, doctors are seeing some cases where the patient's most critical need is a warm bed.

"The more I see these patients, the more I realize that if it's sleeting and raining outside, the emergency room is the only place they have," said Dr. R. Corey Waller, director of the Spectrum Health Medical Group Center for Integrative Medicine in Grand Rapids, Michigan.

Government agencies such as the National Institutes of Mental Health, the Centers for Disease Control and Prevention and the Substance Abuse and Mental Health Services Administration could not provide fresh data on use of psychiatric services in recent years.

But doctors from more than a dozen hospitals nationwide, mental health advocacy groups and state-funded agencies told Reuters they are all seeing a marked increase in psychiatric emergencies.

A WORSENING PROBLEM

The National Association of State Mental Health Program Directors (NASMHPD), an organization of state mental health directors, estimates that in the last three years states have cut $3.4 billion in mental health services, while an additional 400,000 people sought help at public mental health facilities.

In that same time frame, demand for community-based services climbed 56 percent, and demand for emergency room, state hospital and emergency psychiatric care climbed 18 percent, the organization said.

"This wasn't one round of cuts," says Ted Lutterman, director of research analysis at NASMHPD Research Institute. "It was three or four for many states, and multiple cuts during the year."

If the economy doesn't improve, next year could be worse because many community mental health agencies are cutting programs and using up reserve funds, says Linda Rosenberg, president of the National Council for Community Behavioral Healthcare.

"It's been horrible," she said. "Those that need it the most - the unemployed, those with tremendous family stress - have no insurance."

In the emergency room, this increased demand has meant doctors and social workers are spending hours and sometimes days trying to arrange care for psychiatric patients languishing in the emergency department, taking up beds that could be used for traditional types of trauma.

More than 70 percent of emergency department administrators said they have kept patients waiting in the emergency department for 24 hours, according to a 2010 survey of 600 hospital emergency department administrators by the Schumacher Group, which manages emergency departments across the country.

Ten percent said they had "boarded" patients for a week or more.

And many hospitals are not prepared for the increased caseload of psychiatric patients, says Randall Hagar, director of government affairs for the California Psychiatric Association.

California cut $587 million in state-funded mental health services in the past two years, the most of any state, according to the National Alliance on Mental Illness, a patient advocacy group.

"They don't have secure holding rooms. They don't have quiet spaces. They don't have a lot of things you need to help calm down a person in an acute psychiatric crisis," Hagar said.

"Often you have a patient strapped to a gurney in a hallway outside of the emergency department where social workers are desperately trying to find an inpatient bed," he said.

FROM CITIES TO SMALL TOWNS

In North Carolina, the state has cut its inpatient psychiatric capacity by half since 2005, says Dr. Bret Nicks, an emergency physician at Wake Forest Baptist Medical Center in Winston-Salem and a spokesman for the American College of Emergency Physicians.

Nicks points to a report from the Institute of Medicine released in 2006 that found U.S. emergency departments were already overtaxed and overcrowded.

"Now you are adding in patients who are unsafe to leave but yet have nowhere to go," he said. "I consider patients with acute psychiatric needs as really the forgotten patient population in the U.S. right now."

Dr. Stephen Anderson is an emergency department doctor at Auburn Regional Medical Center, a mid-size suburban hospital outside of Seattle.

"When the economy is hurt they are some of the first to drop off the healthcare rolls," he said of local residents in the largely blue-collar community.

Anderson, who heads the Washington Chapter of the American College of Emergency Physicians, said the state has lost a third of its inpatient psychiatric beds in the past decade.

Lately he is seeing a marked escalation in patients with psychiatric problems turning up in the emergency department. In early December, a third of its beds were occupied with people in a psychiatric crisis who were not safe to return to the community.

The problem extends out to small towns.

Sullivan splits his time between the big emergency department at the University of Illinois Medical Center at Chicago and St. Margaret's Hospital, a tiny facility in Spring Valley, Illinois, about 100 miles southwest of the city.

On a recent shift, a young woman with schizophrenia arrived at the hospital. She had just lost her job and apartment and was living with relatives. She could not afford the medications that were keeping her illness in check.

The woman asked Sullivan to switch her prescriptions to drugs that could be found on the $4 discount list at Wal-Mart and other discount stores.

"I didn't feel comfortable doing that," Sullivan said, noting that emergency physicians are being asked to deliver specialized care that should be handled by a psychiatrist.

He found a healthcare facility about 25 miles away with a psychiatrist who could help, but even that presented a problem for the woman, who had no way of getting to the appointment.

"It's almost akin to having a cardiac patient come in and say, 'I need someone to adjust my defibrillator.' In the emergency department, we can do a lot, but there are some things we have to leave with the specialists," he said.

Huge rally increases pressure on Putin

Tens of thousands of Russians jammed a Moscow avenue Saturday to demand free elections and an end to Prime Minister Vladimir Putin's 12-year rule, in the largest show of public outrage since the protests 20 years ago that brought down the Soviet Union. Gone was the political apathy of recent years as many shouted "We are the Power!"

The demonstration, bigger and better organized than a similar one two weeks ago, and smaller rallies across the country encouraged opposition leaders hoping to sustain a protest movement ignited by a fraud-tainted parliamentary election on Dec. 4.

The protesters shouted "Russia without Putin" and "New elections, New elections" as one speaker after another called for an end to Putin's reign.

"Do you want Putin to return to the presidency?" novelist Boris Akunin asked from a large stage. Whistling and jeering, protesters chanted: "No!"

The enthusiasm also cheered Mikhail Gorbachev, the last Soviet leader who closed down the Soviet Union on Dec. 25, 1991.

"I'm happy that I have lived to see the people waking up. This raises big hopes," the 80-year-old Gorbachev said on Ekho Moskvy radio.

He urged Putin to follow his example and give up power peacefully, saying Putin would be remembered for the positive things he did if he stepped down now. The former Soviet leader, who has grown increasingly critical of Putin, has little influence in Russia today.

But the protesters have no central leader and no candidate capable of posing a serious challenge to Putin, who intends to return to the presidency in a March vote.

Even at Saturday's rally, some of the speakers were jeered by the crowd. The various liberal, nationalist and leftist groups that took part appear united only by their desire to see "Russia without Putin," a popular chant.

Putin, who gave no public response to the protest Saturday, initially derided the demonstrators as paid agents of the West. He also said sarcastically that he thought the white ribbons they wore as an emblem were condoms. Putin has since come to take their protests more seriously, and in an effort to stem the anger he has offered a set of reforms to allow more political competition in future elections.

Kremlin-controlled television covered Saturday's rally, but gave no air time to Putin's harshest critics.

Estimates of the number of demonstrators ranged from the police figure of 30,000 to 120,000 offered by the organizers. Demonstrators packed much of a broad avenue, which has room for nearly 100,000 people, about 2.5 kilometers (some 1.5 miles) from the Kremlin, as the temperature dipped well below freezing.

A stage at the end of the avenue featured banners reading "Russia will be free" and "This election Is a farce." Heavy police cordons encircled the participants, who stood within metal barriers, and a police helicopter hovered overhead.

Alexei Navalny, a corruption-fighting lawyer and popular blogger, electrified the crowd when he took the stage. He soon had the protesters chanting "We are the power!"

Navalny spent 15 days in jail for leading a protest on Dec. 5 that unexpectedly drew more than 5,000 people and set off the chain of demonstrations.

Putin's United Russia party lost 25 percent of its seats in the election, but hung onto a majority in parliament through what independent observers said was widespread fraud. United Russia, seen as representing a corrupt bureaucracy, has become known as the party of crooks and thieves, a phrase coined by Navalny.

"We have enough people here to take the Kremlin," Navalny shouted to the crowd. "But we are peaceful people and we won't do that ? yet. But if these crooks and thieves keep cheating us, we will take what is ours."

Protest leaders expressed skepticism about Putin's promised political reforms.

"We don't trust him," opposition leader Boris Nemtsov told the rally, urging protesters to gather again after the long New Year's holidays to make sure the proposed changes are put into law.

He and other speakers called on the demonstrators to go to the polls in March to unseat Putin. "A thief must not sit in the Kremlin," Nemtsov said.

The protest leaders said they would keep up their push for a rerun of the parliamentary vote and punishment for election officials accused of fraud, while stressing the need to prevent fraud in the March presidential election.

Former world chess champion Garry Kasparov was among those who sought to give the protesters a sense of empowerment.

"There are so many of us here, and they (the government) are few," Kasparov said from the stage. "They are huddled up in fear behind police cordons."

The crowd was largely young, but included a sizable number of middle-aged and elderly people, some of whom limped slowly to the site on walkers and canes.

"We want to back those who are fighting for our rights," said 16-year-old Darya Andryukhina, who said she had also attended the previous rally.

"People have come here because they want respect," said Tamara Voronina, 54, who said she was proud that her three sons also had joined the protest.

Putin's comment about protesters wearing condoms only further infuriated them and inspired some creative responses. One protester Saturday held a picture montage of Putin with his head wrapped in a condom like a grandmother's headscarf. Many inflated condoms along with balloons.

The protests reflect a growing weariness with Putin, who was first elected president in 2000 and remained in charge after moving into the prime minister's seat in 2008. Brazen fraud in the parliamentary vote unexpectedly energized the middle class, which for years had been politically apathetic.

"No one has done more to bring so many people here than Putin, who managed to insult the whole country," said Viktor Shenderovich, a columnist and satirical writer.

Two rallies in St. Petersburg on Saturday drew a total of 4,000 people.

"I'm here because I'm tired of the government's lies," said Dmitry Dervenev, 47, a designer. "The prime minister insulted me personally when he said that people came to the rallies because they were paid by the U.S. State Department. I'm here because I'm a citizen of my country."

Putin accused the United States of encouraging and funding the protests to weaken Russia.

Putin's former finance minister surprised the protesters by saying the current parliament should approve the proposed electoral changes and then step down to allow new parliamentary elections to be held. Alexei Kudrin, who remains close to Putin, warned that the wave of protests could lead to violence and called for establishing a dialogue between the opposition and the government.

"Otherwise we will lose the chance for peaceful transformation," Kudrin said.

Kudrin also joined calls for the ouster of Central Election Commission chief Vladimir Churov.

Putin has promised to liberalize registration rules for opposition parties and restore the direct election of governors he abolished in 2004. Putin's stand-in as president, Dmitry Medvedev, spelled out those and other proposed changes in Thursday's state-of-the nation address.

Gorbachev, however, said the government appears confused.

"They don't know what to do," he said. "They are making attempts to get out of the trap they drove themselves into."

Louis Vuitton sues over knockoff bag in "Hangover 2" (Reuters)

LOS ANGELES (TheWrap.com) ? It might be more expensive, but sometimes it really pays to buy name-brand.

French luxury brand Louis Vuitton has filed a lawsuit against Warner Bros. over the use of a Louis Vuitton knockoff bag in "The Hangover: Part II."

According to the suit, filed in U.S. District Court in New York on Thursday, an airport scene in the film makes use of a counterfeit bag made by a company named Diophy, which is currently being sued by Louis Vuitton for trademark infringement.

During the scene, Zach Galifianakis' character, Alan, warns the character Stu (played by Ed Helms), "Careful, that is ... that is a Louis Vuitton." The French company objects to both the movies use of its trademark, and the characterization of the bag as a real Louis Vuitton. According to the suit, Warner Bros. is "explicitly misleading the public about the source of the Diophy Bag" and undermining the company's enforcement efforts against counterfeits of its product.

According to the suit, Louis Vuitton objected to Warner's after the film's opening, but the company still released the film on DVD with the offending footage included.

Citing a previous lawsuit over the film -- during which Warner's said it was considering altering a scene that featured a tattoo that might have violated a copyright -- Louis Vuitton argues that altering the airport scene to eliminate the company's trademarked design would have been an easy fix.

In the suit, the French company is asking that the court permanently enjoin Warner's from using its marks in any way, and order the company to surrender all copies of the film containing the marks. It is also asking for triple damages on all profits that Warner Bros. may have amassed from using Vuitton's marks, plus attorneys' fees and court costs.

This is far from the only lawsuit that "The Hangover Part II" -- a huge hit that has amassed a worldwide take of more than $500 million -- has sparked. In addition to the tattoo lawsuit, which was settled in June, the company was sued in October by an aspiring actor who claims that the plot was stolen from his life experiences.

Warner Bros. had no comment for TheWrap on the lawsuit.

Jay Mohr joins the comedy film "Burt Wonderstone" (Reuters)

LOS ANGELES (TheWrap.com) ? Jay Mohr has landed a role in "Burt Wonderstone," New Line's comedy starring Jim Carrey, Steve Carell, Olivia Wilde, Steve Buscemi and James Gandolfini, TheWrap has learned.

The actor, comedian and "Last Comic Standing" host and executive producer will play Rick the Implausible, a magician.

New Line's movie begins shooting in January. It is about successful Las Vegas magician, Burt Wonderstone, whose partner dies in a risky stunt. After the death, Wonderstone, played by Carell, struggles to rediscover his love for magic and defeat a rival magician.

Don Scardino, who has directed more than 70 episodes of "30 Rock" and many episodes of "Law and Order," "Hope & Faith" and others, is directing.

Jason Reitman wrote the most recent version of the script.

Mohr played Bob Sugar in 1996's "Jerry Maguire," Marty in 2005's "Are We There Yet?," Sgt. Mike Clady in the 2008 film "Street Kings" and Billy in 2010's "Hereafter."

He also starred as Gary Brooks in the 2010 CBS series "Gary Unmarried."

Jay Gray, NBC Reporter, Arrested For DUI After Chasing Jerry Sandusky Exclusive


Proving that NBC personnel suck at holding their liquor as well as producing TV shows and updating their website, correspondent Jay Gray was arrested last Monday after allegedly getting wasted at a party thrown by Jerry Sandusky's lawyer.

Gray was invited by attorney Joe Amendola to watch the New York Giants game the night of December 11, along with reporters from other media outlets, all of whom were vying for an exclusive interview with disgraced former PSU coach Sandusky.

Gray got "really drunk" during the game - won by the Giants over Dallas, 37-34, after an INSANE finish! - and sometimes after midnight, Gray left the party and began driving. Intoxicated. The Pennsylvania State Police stopped and popped him for DUI.

According to police records released after the fact, "He [Gray] had been drinking and was arrested for DUI." The time of arrest was 1:45 a.m. on December 12.

Somehow we're guessing Sandusky, the former defensive coordinator and accused child molester, will be taking his big-ticket tell-all interview elsewhere.

Sony issues apology and software update for first batch of Vitas (video)

The PlayStation Vita has just barely hit the market in Japan, and it's already off to a rocky start with consumers. Only a matter of hours after the first unit was sold, users started complaining of software bugs, unresponsive touchscreens and complete system freezes. It's actually quite normal for new consoles to have a rough first few days with their new owners, but the speed at which it went from a few folks complaining of poor performance to a firestorm of furious customers flooding Twitter was quite surprising. Strangely enough, Sony must have been aware that some of these issues still lurked inside their latest portable gaming hardware, as the company has already issued a public apology and a software update. Hopefully most of the kinks will be worked out by the time the handheld hits stateside -- we're not sure how forgiving American gamers will be when faced with the lag you can see in the video after the break.
